The following documents must be uploaded and attached to the application in order to be submitted.  The only exception is the Teacher Reference, which you can either print out and give to the teacher to fill out, or type in their email address in the appropriate field for the reference form to be emailed to them directly.  Teacher References must be emailed to Cammy Nguyen at cnguyen@holyfamilycs.org  by the February 1st deadline.  For applicants who apply after February 1, the Teacher Reference will need to be emailed to Cammy Nguyen before the application can be reviewed. 

 


Required Documents

A complete application packet containing the following items must be submitted for any student to be considered for admission:

Online Application and $150 Application Fee (non-refundable) — If you applied last year and were added to the wait pool, please email Cammy Nguyen at cnguyen@holyfamilycs.org and she will bypass the fee.

Birth Certificate

Baptismal Certificate

Current Immunization Record signed by a physician

Most Recent Standardized Test Scores (2nd — 8th-grade applicants)

Most Recent Report Card  (1st — 8th-grade applicants)

Teacher Reference Form (emailed to cnguyen@holyfamilycs.org)
